The photo is real and has not been retouched in any manner (except to adjust the brightness and contrast so as to better bring out the stain). The white stain is mold which just happens to have taken on the shape of a leering demonic face. It's a prime example of pareidolia. Nothing more; nothing less. Sorry, ghost chasers!

By the way, my mother has firmly stated that the hymnal is not for sale.
